DENTAL SERVICES

HOPE Family Health is happy to announce that we are able to offer dental services at greatly reduced rates to patients who qualify for our contracted dental services.

Who is eligible?

• HOPE patients who have been seen by one of our providers within the last 12 months; and 
• Those who have a Sliding Fee Discount Scale Agreement signed within the past 12 months; and
• Those who have provided us with a copy of all documentation for household proof of income; and
• Those who fall below 200% of the Federal Poverty Guideline for household size and income.
• If you do not qualify for our contracted dental services, we have great referral relationships with dentists in the area and are happy to connect you with one that will meet your needs.
