HOPE is a nonprofit Federally Qualified Health Center (FQHC), located in Sumner and Macon counties of Tennessee, serving the surrounding areas. HOPE is unique in that we provide a team-based integrated approach based on our patients' individual and unique care needs. What does that mean? Our patients can access a broad spectrum of services under one roof and providers from the various services, coordinate care and services as a team to ensure the highest quality of care and the patient experience.
HOPE serves both insured and uninsured patients with comprehensive and affordable medical services, behavioral health services, out-patient psychiatric care, on-site pharmacy, insurance enrollment assistance, and dental health services for the whole family. Many of our services can also be done through telehealth if desired.
We welcome patients from all walks of life and seek to ensure care is not just affordable but accessible to all. As a result, HOPE offers language interpretation on-site for Spanish, American Sign Language, and many other languages.

HOPE Family Health is happy to announce that we're able to offer many dental services at a greatly reduced rate to our HOPE patients who qualify! These discounted services include: fillings, extractions, cleanings, dental evaluations, x-rays, sealants, and topical fluoride.
HOPE Family Health offers integrated services that enable our patients to easily connect to the services they need and receive help when needed. HOPE offers free Insurance Application Assistance, Prescription Assistance Navigation, Language Interpretation, a direct connect on our website to the patient's Patient Portal and much more.

Our providers treat minor medical problems like colds and the flu and help patients with chronic, but manageable conditions, such as diabetes and high blood pressure. We are often the first to notice signs of a more serious illness or condition because we provide continuity of care by seeing patients on a regular basis for many years and noting changes in the patient's health status from one year to the next.
HOPE Family Health is a recognized PCMH once again in 2021. The patient-centered medical home (PCMH) is a model of care in which patients are engaged in a direct relationship with a chosen provider who coordinates a cooperative team of healthcare professionals, takes collective responsibility for the comprehensive integrated care provided to the patient, and advocates and arranges appropriate care with other qualified providers and community resources as needed.
PCMH practices develop transdisciplinary care teams to improve care coordination and care management of patient populations aiming to improve safety, efficiency, and quality in patient care.
HOPE Family Health has been recognized for Advancing HIT (health information technology) for Quality by the Health Resources and Services Administration (HRSA). This recognition as making “notable quality improvement achievements” with one of HRSA’s new Community Health Quality Recognition (CHQR) badges is an honor for any FQHC. According to HRSA, these badges recognize achievements in access, quality, health equity, and health information technology (HIT) for the 2020 Uniform Data System (UDS) reporting period. We are honored to receive such a prestigious award.
