Position Description – Clinical Pharmacist

EXEMPT/NON-EXEMPT STATUS:  Non-exempt

 

JOB SUMMARY: This position is a pharmacist who works with other providers and clinical staff to oversee initiatives that promote safe medication use by focusing on medication related outcomes and identifying and managing drug related problems. This position will be responsible for managing the Clinical Pharmacy Program, in collaboration with other pharmacists and providers, for HOPE Family Health.

 

PRIMARY DUTIES AND RESPONSIBILITIES:

 

  1. Meeting with patients to assess pertinent patient information and collaborate with other members of the healthcare team, patients, and caregivers to provide value-based, patient-centered care.

  2. Identify possible knowledge gaps and educate patients and caregivers about proper medication use, side effects, potential interactions, and necessary education to minimize potential knowledge gaps related to the patient’s healthcare.

  3. Provide clinical recommendations pertaining (but not limited) to gaps in care, high risk medications, compliance and adherence, drug interactions, therapeutic / generic substitution.

  4. Provides drug information training/education to staff on an as needed basis.

  5. Performs Medication Therapy Management and Comprehensive Medication Reviews patients as needed.

  6. Maintenance of Collaborative Pharmacy Practice Agreement(s) as needed to enhance clinical pharmacy services.

  7. Ensure timely documentation of pertinent clinical data in patients’ electronic medical record.    

  8. Supervise the work of assigned support personnel; advising management on pending issues and needed actions.

  9. Adhere to Infection Prevention policies and protocols that protect patients and staff

  10. Maintain a safe and clean work environment as set in department policy

  11. Establish and maintain positive working relationships with all clinical and non-clinical personnel

  12. Perform job duties contained in Staff Pharmacist Job Description as necessary

  13. Responsible for oversight of pharmacy student and pharmacy resident training programs.

  14. May be responsible for administering vaccines and therapeutic injections in a clinic or pharmacy setting

  15. Performs other duties as assigned to meet departmental objectives and the goals and objectives of HFH

  16. Participates in supporting the organization’s vision, mission, culture and adheres to HFH values

  17. Serves on the Quality Improvement Committee and assists in initiatives set forth by that committee.

 

INTERMITTENT DUTIES:

  1. Perform other duties that may be assigned by the Associate Chief Medical Officer or the Director of Pharmacy Services.

OFF-SITE WORK:

  1. Off-site work is required occasionally.

  2. This position has the option to work off-site with the permission of supervisor. Tasks that may need to be done off-site could include:

    1. Health fairs and speaking engagements

    2. chart reviews, telephone encounters, patient calls, virtual visits

    3.  seminars, trainings, conferences

    4. script data entry and answering pharmacy phone calls remotely

    5. any other items determined appropriate by the Director of Pharmacy Services.

 

QUALIFICATIONS/OTHER:

 

  1. Bachelor’s Degree or Doctor of Pharmacy (PharmD) degree from an accredited pharmacy program is required.

  2. Current and unrestricted licensure as a Pharmacist in the State of Tennessee. 

  3. Strong clinical pharmacy knowledge (BCACP or similar BPS certification strongly preferred)

  4. Commitment to professionalism and a dedicated desire to advance patient care

  5. Demonstrates interpersonal, consultative, and problem-solving skills with a focus on providing exceptional service to our patients.

  6. Excellent verbal and written communication skills

 

TECHNICAL SKILLS:

 

  1. The ability to work as a part of a TEAM – as a part of the pharmacy staff, as well as a part of HOPE.

  2. Operate a motor vehicle with current Class D driver’s license.  Operate a personal computer and calculator.      

  3. Problem Solving:  Frequent problems arise in this position that are outside established policies or bodies of knowledge calling for high-level skills in problem solving.

  4. Decision Making:  Frequent situations arise in this position calling for careful and analytical judgment.

  5. Supervisory:  Extensive responsibility for the direct supervision of management, staff and contract employees.  Requires the judicious use of delegation.

  6. Interpersonal:  Must interact with patients, employees, Board of Directors, governmental officials, and the public.

  7. Working Conditions:  Works in well-lighted, climate-controlled building.  May be subjected to appearance in less than desirable environment and travel in adverse weather conditions.  May be exposed to body fluids.

  8. PHYSICAL REQUIREMENTS: Requires sufficient visual acuity to be able to read and write and operate equipment common to this position.  Required to read, write and speak English with the ability to speak a foreign language helpful.  Requires sufficient hearing level to be able to effectively communicate with people directly or by telephone.  Occasionally lifts items weighing up to 25 pounds.
