About HOPE
HOPE Family Health was founded in 2005 with the mission of providing affordable healthcare to all regardless of insurance status and ability to pay. HOPE is the only faith-based non for profit health center of its kind in this area. We are a Federally Qualified Health Center located in Sumner and Macon counties of Tennessee, treating patients throughout the surrounding area. HOPE Family Health integrates medical health and mental health conveniently located under one roof. Our unique approach to healthcare has brought HOPE to thousands of patients.
HOPE Family Health seeks to improve access to medical and mental healthcare in rural areas, with an emphasis on vulnerable populations such as the uninsured, underinsured, poor, homeless, children, migrant workers, and those addicted to substances. We seek to restore dignity, faith, HOPE, and health in those we serve by making them partners in the healing process and providing them with loving, compassionate care.


Our Vision
In 2005 HOPE Family Health founders envisioned a health center that would never turn a patient away based on insurance status or ability to pay. HOPE is grateful to be able to offer the following programs and supports to help our patients access affordable care:
Sliding Scale Fees for uninsured patients with household Income below 200% of the federal poverty level.
Free Insurance Application Assistance for patients and community members who would like to get the facts about the ACA or get help applying for insurance through the Health Insurance Marketplace, as well as Medicaid and CoverKids.
Prescription Assistance Navigation to help eligible patients apply for free or reduced-cost brand name prescriptions directly from drug manufacturers and other prescription assistance programs.
The HOPE Assistance Fund and HOPE Bridge Programs: Help with critical one-time or temporary needs for patients facing acute medical and financial hardship. Please note: Support to patients through these programs is dependent on the availability of funds. The HAF and Bridge Programs are funded exclusively by donations from congregations and generous individuals just like you!
Payment Plans for One-time Urgent Needs: In the event that a patient has an urgent medical need, as verified by a HOPE provider, and has no ability to pay on the date of service, a one-time payment plan can be arranged.
Our Additional Designations, Funding, Memberships, and Partners
In addition to federal funding and modest clinical revenue, HOPE receives limited but important state funding as a designated Tennessee Safety Net Provider.
