JOB SUMMARY: A medical assistant/licensed practical nurse is a multi-skilled professional and a valuable asset to a provider. They are an integral part of the healthcare team assisting in patient care management. They perform several functions, for instance: administrative medical assistants are tasked to update the patients’ medical record in the Electronic Medical Record.


JOB SUMMARY: The certified mid-level medical provider will function as a member of a team of health professionals providing medical services to HOPE Family Health patients.  The mid-level medical provider shall, under appropriate direction and supervision by a collaborating physician, provide care, assessment, interpretation of data, and develop and implement therapeutic care plans.  The mid-level medical provider may be permitted to perform the following functions.  This is not intended to be all-inclusive or exclusive and shall be directly established by the provider’s privileges as established by HOPE Family Health in the providers contract and credentialing agreement.


JOB SUMMARY: A full-time licensed mental health provider to provide behavioral health services to HOPE patients. The Behavioral Health Consultant (BHC), working in integration with HOPE’s medical providers, will assess, screen, treat, and refer patients to support optimal health functioning in each patient’s life.

The program is designed to provide universal access
to behavioral assessment, and to provide outpatient behavioral health interventions to patients with behavior or substance use concerns.
